Adults’ Services
Achievers Health Care provides structured, person-centred Adults’ Services designed to empower individuals to live confidently, independently, and safely within their communities. We support adults with autism, learning disabilities, emotional wellbeing challenges, behavioural needs, social isolation, and community access difficulties through governance-led and outcome-focused support services. Our approach promotes independence, emotional resilience, confidence building, and meaningful progression while ensuring individuals receive safe, consistent, and professional support tailored to their personal goals and strengths.
Our Independence Support services help adults develop practical life skills such as budgeting, travel training, routines, appointments, communication, and personal organisation that encourage greater self-confidence and long-term independence. Through Community Outreach and Wellbeing services, we support individuals to engage positively within their communities, reduce isolation, improve emotional wellbeing, and participate in meaningful social and recreational activities that promote confidence, inclusion, and personal growth.
We also provide positive and trauma-informed Behaviour Support alongside Employment Readiness programmes that help individuals develop emotional regulation, workplace confidence, communication skills, structure, and progression pathways towards volunteering, education, or employment opportunities. Our services are designed to create safe and supportive environments where adults can build stability, improve wellbeing, strengthen life skills, and achieve positive outcomes that enhance their overall quality of life and future opportunities.
